Payment is not part of this form. Please make payment once the form has been submitted. Your entry will be processed when payment is received.

Player Details
You may enter a maximum of one singles, one doubles, and one mixed doubles. All events will run in round-robin format except OPEN Singles is elimination.
Open and A grade standard players may not enter Veteran events.
Saturday - ALL singles events. Sunday - Doubles events, OPEN singles semi-finals. Monday - Mixed doubles events. OPEN singles final.
(e.g. MAS, LAD, MOS, G12D, XCD, LVD)
First Letter:
M = Men's, L = Ladies, X = MiXed, B = Boys, G = Girls.
Last Letter:
(S)ingles or (D)oubles
In Between:
The GRADE: (O)pen, A, B, C, D, (V)ets, 10, 12, 14, 16 & under
